Responsibilities
Daily duties will include:
- Develop and maintain effective tracking and referral tools;
- Know and collaborate closely with the various partners and resources in the neighborhood;
- Improve and keep up to date the social map of the neighborhood resources;
- Improve and develop management tools to follow the evolution of the project and the care trajectories;
- Improve and develop management tools to monitor the evolution of the project and the care trajectories (dashboard, data, statistics, etc.);
- Raise awareness and mobilize key players around the project;
- Recruit, train and support a network of sentinels to help identify vulnerable seniors;
- Accompany vulnerable seniors in the neighborhood;
- Receive alerts from sentinels and stakeholders in the neighborhood and ensure that the needs are referred to the appropriate resources;
- To offer an active listening to seniors and identify their needs for services and care
- Referring these people to the relevant resources in the community and in the health system;
- To promote partnership and teamwork;
- Work in close collaboration with the CIUSSS to facilitate the management of cases files;
- Participate in the Mercier-Ouest seniors' consultation;
- To be involved in the various work sites concerning geriatric projects throughout Quebec
- Ensure the follow-up and evolution of the projects;
- Inform the steering committee of progress at meetings;
- Produce reports, activity reports and participate in the writing of grant applications;
Qualifications
Do you have what it takes for this position?
- A university degree in social work or in a field related to the project
related to the project;
- Very good knowledge of the issues of mental health, loss of autonomy, social exclusion, abuse and social isolation among seniors;
- Excellent interpersonal skills, empathy, ability to listen, ability to deal with complex situations, ability to set limits;
- Leadership, organizational skills and excellent teamwork skills;
- Excellent command of French (oral and written);
- Good knowledge of English (oral and written);
- Experience in project implementation or coordination;
- Experience and/or knowledge of the community environment, the health and social services network;
- Valid driver's license and car (a major asset and facilitator)
