This role is based in Toronto and follows a hybrid work model, offering a combination of in-office collaboration and remote flexibility. We are seeking a Registered Nurse who is available for full-tme hours, is fully bilingual in English and French, and brings experience in managed care or acute care environments.

Responsibilities
Bilingual Nurse Case Manager - Downtown Toronto, Hybrid - Healthcare
Conduct clinical assessments using real-time and historical medical data
Manage care coordination and ensure services align with clinical criteria and policy coverage
Perform utilization review (UR) and utilization management (UM) functions
Support hospital admission reviews, determining medical necessity and appropriate length of stay
Develop and oversee patient treatment plans in consultation with healthcare providers
Coordinate second opinions, diagnostic testing, transfers, and discharges as required
Manage the full scope of medical repatriation or evacuation planning and execution
Escalate complex or non-standard cases to the supervising physician or medical director
Document all case information in the case management system accurately and in real time
Maintain clear, professional communication with providers, internal teams, and clients
Participate in quality assurance initiatives, internal training, and team meetings
Qualifications
Bilingual Nurse Case Manager - Downtown Toronto, Hybrid - Healthcare
Registered Nurse with a Bachelor of Science in Nursing (BScN)
Active, unrestricted license with the College of Nurses of Ontario (CNO)
Minimum 4 years of experience in managed care or acute care (ER, ICU, CCU, etc.)
Fluency in English and French is required
Strong interpersonal and communication skills
Moderate proficiency in digital tools and case management systems
Highly organized, with the ability to manage multiple cases simultaneously
Ability to work independently and within a team-based environment
Experience in culturally diverse or international settings is an asset
Comfortable supporting non-clinical team members with clinical knowledge and training
